Abstract
Purpose: The purpose of this study is to compare the results of the arthroscopic reconstruction of anterior cruciate ligament (ACL) using bone-patellar tendon-bone autograft and hamstring autograft under transtibial technique.

Materials and Methods: We included 73 patients in this study who underwent arthroscopic ACL reconstruction using autologous hamstring tendon (41 cases) and autologous bone-patellar tendon-bone (BPTB) (32 cases) from November, 2005 to October, 2007. The mean followed up period of hamstring tendon group was 29 months (range 24~40 months) and patellar tendon group was 37 months (range 30~46 months). All surgery were done using transtibial technique by a single expert surgeon. Preopertively and at the time of the postoperative 1 year follow up, all patients in each group were evaluated the results of the anterior stability by Lachman test and Pivot shift test, and the activity level and functional status by Lysholm score, Tegner score and IKDC score.

Results: At the time of postoperative 1 year follow up, both group showed improved results in the aspect of anterior instability, activity level and functional status. However, there was no significant differences between the groups.

Conclusion: Arthroscopic ACL reconstruction using both the autologous hamstring tendon and the patellar tendon under transtibial technique during their postoperative 1 year follow up period demonstrated excellent results, but showed no statistically significant difference between the two groups.
